It’s not I remember looking it up once and I think it was about a 3% increased chance of having birth defects which is not very significant. What is a problem is when people keep on marring first cousins over many generations as is common in places like Pakistan, then they have very high rates of birth defects/genetic disease

The closer you are related the higher is the persentage of your own genes passed on, but the higher is the risk of diseases due to ill rezessive alleles. The optimum is around third cousin. In fact if I remember correctly there have been studies that showed that humans found the smell of their third cousins most attractive compared to other relatives.
